Case Manager

Our client is seeking a Case Manager in Camden, NJ.

Earn $13.00 to $14.00 per hour at this full-time, temporary position. Work 40 hours per week, Monday through Friday, 8:30 AM to 5:00 PM.

Job Description:

  • Assist individuals and families find support and assistance they are in need of and qualify for.
  • Reach out to the clients via phone, email, and letters as well as meet face-to-face and interview to assess their service needs.
  • You will develop a plan of service and refer the client to the various programs and services that are available such as medical services, employment services, mental health services, DYFS, etc.
  • Once the plan is in place you will be required to follow up with each service to ensure the client is attending
    and complying with agreements.
  • Reporting and follow up are extremely important.


Requirements:

  • Bachelors Degree in the Human Services field
  • Past employment with Social Services or Human Services
  • Excellent communication skills both written and verbal
  • Excellent computer skills and data entry with knowledge of MS Outlook, Word, Excel, etc
  • Ability to multi-task in a fast-paced environment and follow up on all aspects involved in the designated plan.
  • Hours are Monday through Friday 830 AM until 5:00 PM and may require flexibility should a client not be able to meet during normal business hours.
